Attentive HHS provides skilled nursing and therapy services to homebound patients suffering from acute, chronic, or terminal illness; physical or cognitive disability; and other conditions that require skilled nursing and related services that cannot be provided effectively by family or friends. Our nurses and therapists work closely with physicians, patients, and their families to develop and implement treatment and education programs designed to meet the specific needs of individual patients and their families.

We at Attentive are here to help you. Each patient is assigned a case manager who implements the plan of treatment developed by the physician, and supervises all care provided to the patient.
